- Average case management ratio approximately 25:1
- EMR Systems: Meditech and Midas
Required Experience
Candidates must have:
- Recent acute care hospital case management experience
- Experience with Meditech EMR
- Experience using Midas
- Strong discharge planning and care coordination background
Preferred Experience
- Complex discharge planning experience
- Ability to manage high-volume caseloads in a fast-paced environment
- Strong communication and interdisciplinary collaboration skills
