Medovent Solutions is seeking an experienced Medical Social Worker for an acute care hospital assignment. This role focuses on care coordination, discharge planning, patient advocacy, and connecting patients and families with appropriate community resources and support services.
Position Overview
The Medical Social Worker collaborates with interdisciplinary teams to support safe transitions of care, address psychosocial barriers, and assist patients with complex discharge and community resource needs within an acute care environment.
Required Experience & Knowledge
Candidates should have experience with:
- Acute care hospital social work
- Care coordination and discharge planning
- Biopsychosocial assessments
- Crisis intervention and patient advocacy
- Development of patient-centered plans of care
- Collaboration with interdisciplinary healthcare teams
Community Resources & Support Services
Knowledge of community-based programs and resources preferred, including:
- Medicaid and Medicare
- Transportation assistance programs
- Housing and community support resources
- Medication assistance programs
- Financial and income-based assistance services
- Home health and community-based services
Clinical & Psychosocial Experience
Preferred experience includes:
- High-risk patient interventions and safety planning
- Advance directives and guardianship processes
- Adult and child protective services involvement
- Substance use and behavioral health support
- Suicide/self-harm risk intervention
- Complex discharge planning
Acute Care Experience Preferred
Experience working in:
- ICU settings
- Medical-Surgical units
- Short-term acute care hospitals
Key Responsibilities
- Coordinate discharge plans and post-acute services
- Facilitate referrals to rehabilitation, long-term care, and community agencies
- Participate in interdisciplinary care conferences
- Document patient interactions and care coordination activities
