Position Summary
Medovent Solutions is seeking an experienced Inpatient Clinical Documentation Specialist to support accurate, complete, and compliant clinical documentation. This position works collaboratively with physicians, coding professionals, and other members of the healthcare team to ensure the medical record accurately reflects each patient’s clinical condition, severity of illness, risk of mortality, and services provided.
The successful candidate will have extensive inpatient clinical documentation integrity experience and proficiency with Optum Computer-Assisted Coding. The hospital’s diverse patient population includes patients receiving burn care and gender-affirming procedures.
Key Responsibilities
- Conduct concurrent inpatient medical record reviews to identify opportunities for documentation clarification and improvement.
- Ensure clinical documentation accurately reflects patient acuity, severity of illness, risk of mortality, and the resources used to provide care.
- Initiate compliant physician queries when documentation is incomplete, unclear, conflicting, or clinically inconsistent.
- Collaborate with physicians, coding professionals, nursing staff, and other healthcare team members to support documentation accuracy and completeness.
- Apply clinical knowledge and established coding and documentation guidelines when reviewing inpatient records.
- Review complex cases across a range of specialties, including burn care and gender-affirming procedures.
- Maintain an expected productivity level of at least 30 reviews per day, excluding retrospective reviews.
- Meet established standards for review quality, query compliance, productivity, and timely case completion.
- Participate in education, performance-improvement initiatives, and ongoing clinical documentation integrity training.
- Maintain patient confidentiality and comply with all applicable regulatory, privacy, and organizational requirements.
Required Qualifications
- At least four years of inpatient clinical documentation integrity experience.
- Professional experience using Optum Computer-Assisted Coding.
- Strong knowledge of inpatient clinical documentation integrity practices, compliant query standards, and coding-related concepts.
- Ability to consistently complete at least 30 reviews per day, excluding retrospective reviews.
- Strong clinical judgment, analytical ability, attention to detail, and written and verbal communication skills.
- Ability to work effectively with physicians, coding professionals, and multidisciplinary healthcare teams.
Preferred Qualifications
- Active Registered Nurse credentials preferred; qualified Foreign Medical Graduates will also be considered.
- Certified Clinical Documentation Specialist certification preferred.
- Experience using the Cerner electronic medical record system.
- Experience supporting inpatient clinical documentation within a large or multi-facility healthcare environment.
- Experience reviewing complex burn-care cases or documentation related to gender-affirming procedures.
